In-Home Assessments
After a level of care is agreed upon, but before your loved one begins services, our dedicated on-call nurse will visit the home to gather all pertinent medical and personal information, create a customized care plan based on this information and perform a fall risk assessment.- The ComForcare Care Plan addresses both the physical and social needs of every client requesting our services. We believe that this approach allows us to deliver a customized care plan that is unique to each individual receiving our service.
- During the assessment, the nurse will also review the proposed schedule of service and answer any further questions that you or your loved one may have.
- During the time a ComForcare RN or Staff Member is conducting the assessments, we are also able to provide invaluable geriatric care consultation. Often times, for example, we are asked what local Skilled Nursing Agency or DME (Durable Medical Equipment) provider would be also helpful to supplement care for an individual.
- If the family doesn’t already have one, the ComForcare staff member will create a Medication List including current medicines, dosages, reasons for taking, and even contact information of local physicians and pharmacies. A copy of this, along with the entire care plan, is given to the client at the end of the assessment.
- An up-to-date Emergency Contact List is also developed during the assessment. This will list out all relevant emergency names, telephone numbers, e-mails etc. of both medical personnel, family members, friends, and other supplemental health care providers. This list could be invaluable (along with Medication List) to family members.
- In North Carolina, state licensed In-Home Care Agencies such as ComForcare are required to perform re-assessments for clients every 60 to 90 days. During these random visits, we also review the performance of our own Caregivers.
