Forms / Documents for Existing ComForcare Caregivers / Employees

Please feel free to download a “ComForcare” flowsheets for use with your clients. As a reminder, please first choose the correct/appropriate flowsheet depending upon your client. Also, print out at least 3 copies: 1 to be left with Client, 1 to be mailed back to Office, and 1 for the Caregiver.

A couple of reminders regarding flowsheets. These items have been explained to all ComForcare employee’s during orientation, but as a reminder:

• Even if you fax or e-mail a copy of your signed flowsheet to the office, you MUST mail to the office the original flowsheet signed. Failure to do so can result in wages be taken back out of future paychecks.
• On all flowsheets, please try and the both client initials per each day of service and a signature at the end of the week. Also, you must report exact actual times for arrival and departure for each service day. Failure to turn in a flowsheet without a client’s original/actual signature will result in deduction of wages or non-payment of wages on your next paycheck.
• Signed Flowsheets, for prior week of service, is due by the following Monday by 12 Noon. If your last shift, for a particular client, is over Sunday night at midnight, then you are still responsible in getting the flow sheet in by 12 Noon on Monday (next day). Postal service delays/delivery is not an excuse for turning in your timesheets late. You can mail them in (on-time), fax/e-mail them in (and mail originals later), or drop them off in-person to office.
